Patient appointment form

Use this form to request an appointment (new patients, existing patients and patients who have seen us previously).

You may email relevant letters or reports separately to us here.

Allergy vaccine reorder request form

Use this form to request an allergy vaccine order (existing and previous patients only).

At present, vaccine supply is stable and can take 3-6 weeks.

Clinical support request form

Use this form to request clinical support (existing or previous patients only). Clinical support may relate to your anaphylaxis or allergy action plan, or prescriptions (including Xolair renewals).

If you wish us to issue a prescription or an allergy/anaphylaxis action plan outside a consultation, we charge a $25 fee. We do not charge for simple electronic queries in respect to allergy vaccine ie. desensitisation or immunotherapy treatments.
